PAYMENT &  INSURANCE 
 
Full payment is due at the time of service. We are "out of network providers" for BC/BS, Aetna, United and Cigna PPO plans. This means we request full payment at the time of service. We electronically file your insurance claim; upon your insurance reimbursement, your check is fowarded to you on the same business business day. Please bring your insurance card and driver's license.
 
Self-pay patients are of course welcome. Payment is required at the time of service. 
 
  • We accept cash, debit card, MasterCard,Visa and Discover.
 
  • We are conveniently locate across the street from Amscot Bank which can cash personal and third party checks as well as provide cash advances.
 
  • We do NOT accept Medicaid, Medicare, HMO's or any PPO other than those listed above. Patients covered by these plans are of course welcome, and payment is required at the time of service. You will be provided an insurance claim form (1500) with all appropriate codes so that you may submit this to your insurance company for your direct reimbursement. 
 
 
FEES
 
Urgent Care, New Patient: $99.00. This represents the fee for a standard 20 minute visit. Complicated and lengthy consultations are priced accordingly.
 
  • New patient visits range from $99.00 to $299.00 based upon the complexity of your problem and the amount of time required for your consultation.
 
  • Medications, Lab Testing and Procedures (such as suturing and splinting) are additional. Patients are informed of all costs before proceeding with any additional expense.
 
Established Patients:
 
  • Follow-up visits range from $40 to $250, again based upon your needs.
 
  • Procedures are followed by a free visit to assure quality control and address any problems.
 
  • Medications, Lab Testing and Procedures are additional. Patients are informed of all costs before proceeding with any additional expense.
